Applying For Service

To apply for service with Access Calgary, an application form must be filled out and an assessment interview must be conducted. Access Calgary conducts interviews with potential customers to determine their eligibility for service and to assess their individual transportation requirements. You may download an application form online, in Portable Document Format (PDF) or Microsoft Word format.

To use the PDF form, you will need an Adobe Reader. If you do not already have an Adobe Reader installed on your machine, you may download a copy.

Interview Locations

Applicants for Access Calgary (shared-ride Handi-Bus and taxis) attend a face-to-face interview as part of the eligibility process. Access Calgary staff are available to meet with applicants at a number of locations throughout the community.

You will meet one of our Eligibility Specialists to talk about your transportation needs and how you get around now. The information gathered in the interview and a review of your application will help us to determine if you are eligible for Access Calgary service. The eligibility process creates a more equitable and integrated system to meet customers' needs based on a range of transportation services available in the community.
